CEO Sydney Enrolment Forms      Top

The CEO Sydney has created the following documents

	Application for Enrolment Form
	New Enrolment - Administration Checklist.

They have kindly permitted us to put them on our web site.

The documents are in MS Word and can be saved to your computer.

Generic Enrolment forms      Top

Two generic enrolment forms that you can save are.

	Enrolment Form
	Detailed Enrolment Form

These documents are in MS Word 97 format. They have been designed using tables which should allow the easy deletion of unwanted text or the addition of extra text.

If printed on a good quality laser printer there should be a 'wash' over the entire page except for the boxes for hand written information - the information you are collecting.

A separate page will need to be included to help your families to fill in the form correctly. It may also contain lists for the family to select from, e.g. the travel method field may need a list to guide the parents in selecting the valid form of travel.

The separate page should also contain any parental consent that the school requires.

Saving the Enrolment Forms      Top

If you use Internet Explorer as your browser

	Click on the document name and it will open in a new browser window.
	Select File > Save As, to save the form on your computer

If you are using Netscape

	Click on document name
	The option to open the form in a new window or to 'save it to disk' will appear.
	Select the appropriate option.
